Professional Development - Mentoring
If you are interested in being an instructional mentor, contact your building administrator for approval. Please note that this is a two year commitment. Upon administrator approval you will need to have completed three required trainings:
- Introduction to Mentoring
- Coaching and Observation
- Online Observation Tool
If you plan on being an active mentor this school year and have not taken these, you must sign up. Contact Nancy Ziech, confidential secretary for Professional Development, at the Administration Office. 815-577-4032 nziech@psd202.org

Mentor Assignment Form for Building Principals (MS Excel 2007 file)
Please note: This form is due by August 1 in order to prepare for the Mentor meeting during teacher induction. At the end of the first semester this form is also due indicating any changes in assignements. The final submission of this form is due by April 30 for payroll purposes. Please submit to Nancy Ziech , confidential secretary assigned to Professional Development.

Roles and Goals
This form is due twice a year, December 17, 2010 and again by May 6, 2011. Forms should be dated AND initialed by the principal or designee, teacher and mentor. Plans should be turned into the building principal. One copy will be kept by the principal for their files and one copy should be retained by the teacher. The principal will send a copy of all Induction plans to Nancy Ziech, confidential secretary for Professional Development at the Administration Office.
All goals should be addressed by the May submission.

Mentor Forms
The following forms may be used in your observation of mentees. Year 1 teachers must be observed three times a year and Year 2 teachers must be observed twice a year.
